At your first visit you will fill out comprehensive intake forms on your health history, family history and current
medical issues as well as insurance information. This can be done in the comfort of our waiting room while
sipping a cup of tea from our tea selection or prior to your visit by having our office mail them to you. After
this, your practitioner will review the forms and your appointment begins…
