As a courtesy to you, we will bill your insurance company for our provided services. Our practitioners are
credentialed with most major, and many smaller insurance companies. Doctors Donovan and Fahoum and
nurse practitioner Lise Anderheggen-Leif, are listed as primary care providers with most insurances in the State
of Washington. George Whiteside is listed as a preferred provider. Plans and benefits are different with every
company and every plan within a company. This is why it is your responsibility to call your insurance to clarify
benefits and coverage before seeing one of our providers.
For help clarifying coverage and your benefits, please read the following sections to better understand types of
insurance, the language of insurance and some definitions of coverable services according to most plans. While
this is in no way complete or can guarantee coverage for our services, it is an attempt to help educate and
empower you. It is to your advantage to understand your insurance benefits. This will always help to avoid
